Scope
Design a comprehensive Case Management System and Initial Intake Questionnaires within the Well Family System platform to support healthcare providers in streamlining client onboarding, case tracking, and personalized care planning for maternal and child health initiatives.
Process
Collaborated with healthcare coordinators, intake specialists, and administrative teams to map out real-world workflows and identify system bottlenecks. Conducted journey mapping for the intake and case management processes to ensure critical data points were captured efficiently. Developed intake form logic flows to minimize user burden while maintaining compliance with healthcare regulations. Created detailed wireframes and high-fidelity prototypes in Figma for dynamic forms, case tracking dashboards, and client profile views. Conducted usability testing with clinic staff to validate ease of data entry, editing, and case review processes.
Solution
The newly developed Case Management System enabled healthcare providers to efficiently intake new clients, manage ongoing cases, and track key milestones through an intuitive dashboard. Smart intake forms dynamically adapted based on user responses, reducing onboarding time by 30%. The integrated case tracking tools provided clearer visibility into client needs, appointment histories, and community resource referrals, ultimately enhancing the quality of maternal and child healthcare delivery across partner organizations.
